How does the US Presidential nomination work?

I live in Canada and hear all about these democrat and republican debate, but do not understand how it works. I know that currently, Barack Obama and Hillary Clinton are leading the democrats, and John McCain and Mitt Romney are leading the republicans, but how and when do they choose the winners for each party, or do people have a choice of which party candidat to vote for in the election?
